When placed professionally, tooth gems are bonded to the outside surface of the tooth using dental materials. The tooth is not drilled or permanently altered for placement.

However, damage can happen if a tooth gem is placed incorrectly, if non-dental glue is used, or if someone tries to remove the gem at home. That is why we do not recommend DIY tooth gem kits or using household adhesives.
